General information sheets will help you identify weekly tuition options and pricing, your AYS program’s entry door, contact information, hours of operation and program basics. Please keep a copy of your general information sheet handy. Please note, these are subject to change.

Re-enrollment is required for each child per school year. Please complete the 2019-2020 online enrollment form below. You can enroll up to five children with this form. Please include immunization records, Plan of Care and/or Medication Consent forms, any agreements/decrees that impact custody of the child, and payment. Missing information or payment will cause delays.
You will be asked to select your AYS program location and to provide a start and end date, emergency & authorized pick-up contacts, health information, Plan of Care/Medication Consent forms (if applicable), a copy of any agreements/decrees that impact custody, and immunization records for each child. Please note, you will not be able to complete enrollment without uploading immunization records. A credit card payment is required for enrollment fees. Enrollment fees are non-refundable.
Enrollment Fee: $35 per child
Once your online enrollment form is submitted, you will receive an email confirming your submission.

To have your weekly tuition automatically withdrawn from a bank account or credit card, please complete the appropriate form below. Please email this form directly to our business office at email@example.com.
AYS offers a variety of payment options to meet your needs. AYS recommends that you sign up to have tuition automatically deducted from your bank account or credit card. Please complete and submit the appropriate form with enrollment – Bank Account Authorization Form or Credit Card Authorization Form. Online payments can be made through MyProcare. How do I change my AYS plan?
Please complete our online Change of Plan Form. A minimum one-week notice is required for all schedule changes. Schedule changes may be made before fall break. After fall break, a $15 change fee will be applied for each schedule change. Any added days must be approved in advance by the program director. To withdraw your child from AYS, please complete our online Withdrawal Form.
How do I contact my AYS program director?
You can easily locate program contact information via the Locations page. Simply click on your school district/type, then find your school on the following page. The contact information for your program will be located under the school name. You can also find contact information on the General Information Sheet.
How do I find out in which schools AYS operates?
AYS offers over 40 out-of-school time programs in Central Indiana. For a full list, please consult the 2019-2020 Parent Handbook.
How do I apply for Financial Assistance?
AYS accepts CCDF vouchers and recommends that families who meet the guidelines for CCDF vouchers, apply. Visit the www.intakechildcare.com to find out if you qualify and for information on voucher application steps.
AYS has limited financial assistance available on a first-come, first-served basis to families who are at or below 180% of the Federal Poverty Level, and uses the FSSA Child Care Income Eligibility Determination & Fee Schedule to determine assistance eligibility. At this time, all assistance funds for the 2019-20 school year have been awarded.
Enrollment for next school year will open in early spring. We encourage you to enroll and submit financial assistance application for next school year early for the best chance at receiving assistance.
